These online application forms are completed by the LPHA entering or completing the Voluntary Accreditation Program:

Application Form	To apply for the Voluntary Accreditation Program
Performance Standards Form	To submit self-assessment scores for the Performance Standards
Agency Infrastructure Form	To submit self-assessment scores for the agency infrastructure
Workforce Standards Form	To submit self-assessment scores for the workforce standards
Improvement Plan for Accredited LPHAs	For those LPHAs who receive accreditation, this form identifies their plan for continuous quality improvement between initial accreditation and re-accreditation
Interim Plan of Action for LPHA in Conditional Accreditation	For those LPHAs who receive conditional accreditation, this form identifies how they will correct the deficiencies found at the On Site Review Team visit.
